Heat and The Heart
For most of human history, heat was not understood as a health intervention. The Finns built saunas. The Romans built thermae. In Japan, people gathered in sentō. Across Mesoamerica, the temazcal was a place for heat and steam. Different cultures arrived at variations of the same practice of entering heat and then returning to the world outside. Only much later did we begin measuring what happens inside the body while we do it. There are many ways to understand the appeal of heat. It has been used for gathering and solitude, ceremony and recovery, cleansing and rest. Over time, these pages will explore some of them. For now, we follow the heart.
The Cardiovascular Response
A sauna presents the body with a simple problem. Our body needs to remain close to 37°C while the surrounding air may be approaching 90°C. The response begins almost immediately. Blood vessels near the skin widen. Circulation shifts towards the surface. Sweat begins to form. The heart beats faster, moving blood through the body to help release excess heat. From the outside, very little is happening. Inside, the body is working to keep things exactly as they were. In one experimental study, 102 participants spent 30 minutes in a sauna at 73°C. Heart rate increased from an average of 65 to 81 beats per minute. Arterial stiffness decreased, as did systolic and diastolic blood pressure after the session. Other experiments at higher sauna temperatures have recorded cardiovascular responses comparable to submaximal exercise. The comparison is useful, but only to a point, since sitting in a sauna is not exercise. Muscles are not working in the same way and the adaptations are not interchangeable. The heart, however, is certainly not resting.
The Finnish Habit
Some of the most compelling research on sauna comes, unsurprisingly, from Finland. There, sauna is less a health practice than an ordinary part of domestic life. For generations it has been a place for washing, warmth, conversation and solitude. This also makes Finland an unusually interesting place to study what happens when exposure to heat is repeated not for a few weeks, but across a lifetime. Researchers at the University of Eastern Finland followed 2,315 middle-aged men for a median of 20.7 years. Those reporting two to three sauna sessions per week had lower rates of fatal cardiovascular events than those using a sauna once a week. Among men reporting four to seven sessions, the association was stronger still. It is the kind of finding that produces a very tempting headline, but the reality is more interesting.
Association is not cause
The Finnish study was observational. It can tell us that frequent sauna bathing and lower cardiovascular mortality appeared together. It cannot tell us with certainty that one caused the other. And sauna in Finland rarely exists in isolation. It belongs to a broader way of living, shaped by climate, routine, social life and generations of repetition. Separating the effect of the hot room from the life surrounding it is difficult and that distinction matters. Science around heat exposure is promising, but it remains science in progress. The strongest long-term findings describe associations. Smaller experimental studies help explain why those associations might exist by showing measurable changes in blood pressure, arterial function and circulation.
A controlled stress
The body spends much of its life maintaining remarkably narrow boundaries. Temperature is one of them. Heat deliberately disturbs that balance. The body responds by widening blood vessels, increasing blood flow towards the skin, raising heart rate and producing sweat. Once the heat disappears, those systems begin moving back towards baseline. Stress. Response. Recovery. This is one reason researchers are interested in repeated heat exposure. Small, temporary physiological stresses can provoke adaptations that make the body better prepared for the next exposure. The principle is known as hormesis. Exercise works through a related idea. The stress itself is not the objective. Adaptation happens because the body has been given something manageable to respond to. With sauna, exactly how much of this translates into long-term cardiovascular protection is still being worked out. But the immediate response is clear.
Repetition over intensity
The research on sauna offers little reason to make suffering the metric. The landmark Finnish study found stronger associations with greater frequency and longer sessions, but it did not establish an optimal temperature, duration or protocol. Nor does an association mean that extending a session indefinitely will extend its benefit. What stands out instead is something less dramatic. Regular use. For many of the Finnish participants, sauna was not a protocol performed for a season. It was simply something they did, week after week, year after year. The interesting variable may be less about intensity than repetition.
Practice
There is no universally established sauna prescription for cardiovascular health. Most of the long-term evidence comes from traditional Finnish sauna use, while experimental studies vary considerably in temperature and duration. A simple practice sits comfortably within the conditions commonly studied: 80-100°C 15-20 minutes 3-4 sessions weekly Allow the body to cool between rounds. Replace lost fluids. Contrast therapy (alternating between heat and cold) may further enhance vascular responsiveness, though dedicated long-term data is still emerging. Leave the heat if dizziness or discomfort develops. Stay long enough to get warm, but not to prove anything.
